Magandang araw po!
I am Alyssa Mae T. Rivera, 27 years old, single, and a resident of Unit 3C, Primrose Residences, 12 West Capitol Drive, Barangay Kapitolyo, Pasig City 1603. I have been living in the barangay since June 2019, or about seven years, and I am a registered voter of Precinct No. 1287A.
I respectfully request a Barangay Clearance for the following purposes:
- Business permit. I am applying for a Mayor's Permit with the Pasig City Business Permit and Licensing Office for my small online bakery, Mae's Ensaymada Kitchen, which I will run from my unit. The business is registered with the DTI under Business Name Certificate No. 5823144, issued on 12 October 2026.
- Employment. I will also submit a copy to my employer, Northbridge Shared Services Inc., as part of my pre-employment requirements for regularization.
I declare that I have no pending case or complaint in the barangay, and that my business will not cause noise, smoke or obstruction to my neighbors.
Attached are the following for your office's evaluation:
- Photocopy of my Philippine Identification Card (PhilSys) No. 6620-1185-4473-2091
- Community Tax Certificate (cedula) No. CCI2026 08814572, issued at Pasig City on 15 January 2026
- DTI Business Name Registration Certificate
- Contract of lease for my unit, valid until 31 May 2027, with the owner's consent to operate a home-based business
- Two (2) recent 1x1 ID photos
I am ready to pay the clearance fee and to appear at the barangay hall for any interview or verification you may require.
