Special power of attorney
Starting point, not legal advice. Print on long bond paper (8.5 x 13 in), and have it signed by the principal and two witnesses on every page and notarized in front of a notary public, with valid IDs. Banks, the LTO and the Registry of Deeds often have their own SPA forms or ask for specific wording. If you sign abroad, have it notarized and apostilled, or acknowledged at a Philippine Embassy or Consulate. Delete this note before printing.
KNOW ALL MEN BY THESE PRESENTS:
I, ROSALINDA T. GONZAGA, of legal age, Filipino, widow, and residing at 45 Sampaguita Street, Barangay Lahug, Cebu City, Cebu 6000, do hereby name, constitute and appoint my son, ARNEL G. GONZAGA, of legal age, Filipino, married, and residing at Blk 3 Lot 21, Pacific Grand Villas, Barangay Marigondon, Lapu-Lapu City, Cebu 6015, to be my true and lawful attorney-in-fact, for me and in my name, place and stead, to do and perform the following acts:
- To claim, receive and sign for my pension checks, statements and any documents from the Social Security System and the Government Service Insurance System;
- To transact with Bank of the Philippine Islands, Lahug Branch, regarding my Savings Account No. 4189-0337-52, including to withdraw, deposit, request statements, update my records and close the account;
- To transact with the Land Transportation Office (LTO), Cebu City District Office, regarding my motor vehicle, a 2019 Toyota Vios with Plate No. GAB 4721, MV File No. 0722-00000418836, including to renew its registration, secure certificates and process its transfer of ownership;
- To sign, execute and deliver all receipts, forms, applications and other papers needed to carry out the acts above.
HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority to do and perform every act necessary to carry out the foregoing powers, as fully as I could do if personally present, and HEREBY RATIFYING AND CONFIRMING all that my attorney-in-fact shall lawfully do by virtue of these presents.
IN WITNESS WHEREOF, I have hereunto set my hand this 28th day of October 2026 in Cebu City, Philippines.
