Dear Ms. Ocampo,

Good day!

I am writing to formally request a Certificate of Employment (COE) for the following:

Purpose

I will use the COE for my housing loan application with the Pag-IBIG Fund and my tourist visa application with the Embassy of Japan. For these purposes, I kindly ask that the certificate state:

1. My full name, position and date of hire, and that I am still connected with the company
2. My current monthly basic salary of ₱42,500.00 and my monthly allowances of ₱3,000.00
3. That I have been granted vacation leave from 14 to 22 December 2026 and will report back to work on 23 December 2026

If it is company policy to show salary only on request through a separate form, please let me know and I will fill it out.

Timeline

Under DOLE Labor Advisory No. 06, Series of 2020, a certificate of employment should be issued within three (3) days from the employee's request. I would be grateful if the signed COE could be ready on or before Friday, 30 October 2026, as my loan papers are due on 3 November 2026.

I can pick up the original copy at the HR office, or you may email a scanned copy to [email protected] while I wait for the original.
