Letter of authorisation to collect a driving licence
I, Siti Aminah binti Yusof (NRIC No. 880521-10-XXXX), hereby authorise Mr. Rajesh a/l Gopal (NRIC No. 860307-08-XXXX) to act on my behalf to collect my renewed Competent Driving Licence (CDL), classes B2 and D, from the JPJ Wilayah Persekutuan counter.
I am unable to collect it in person because I am travelling for work in Kota Kinabalu, Sabah, from 27 October to 7 November 2026.
